Dementia Support

Our Dementia Support team is providing vital support and a listening ear to people affected by dementia. We are working with people to identify how we can help them during this time by making regular welfare phone calls, ensuring that we maintain that crucial line of communication between ourselves and those who need us.    

We are still actively attending MDT (mutli-disciplinary team) meetings via video conference and taking referrals from clinics, so that people are able to access the support they need.  We have even started doing assessments via video conference where our Dementia Support Coordinators will assess a person’s circumstances and discuss with them what the best way to move forward is. If necessary, we will then involve our Support At Home team who can implement a package of care if this is what the person would like to do. 

As well as all of that we are still taking calls on the 24 Hour Kent Dementia Helpline, helping people in crisis situations and providing advocacy for people living with dementia.  We have also been putting together activity packs for those who use The Beacon or other activity groups, so that they are able to stay stimulated and engaged whilst they are unable to attend our services.
